- Copy of legal entity registration certificate from the Ministry of Justice
- Form attested by a stamp and a signature of company leader indicating legal address and bank information (legal address, name of the bank, bank address, account, individual tax payer number, BIC, code of RTI)

- Copy of the passport of the representative (with registration)
- Stamp of organization
- Documents on assignment of the company leader*
